It probably dates back to 1289 and was rebuilt in its present form around the first half of the fourteenth century by the order of the Celestines. Although the architectural structure is of Romanesque origin, there were numerous interventions and subsequent additions, starting from the fifteenth-century portal (1432), for some realized by Donato Bramante design, up to the apse and the bell tower whose construction is documented in the middle of the seventeenth century (1655 and 1658). A destination for scholars and connoisseurs who traveled on the Via Flaminia since 800 is the Tiranni chapel. Commissioned to Giovanni Santi by Pietro Tiranni, it houses a fresco unanimously considered by the critics as the masterpiece of the Urbino. It is a Sacred Conversation surmounted in the lunette by a Resurrection of Christ. The work is to be traced back to the last period of the Santi's career around the fifties of the fifteenth century. Tradition identifies in the angel's face on the left, on the sides of the Virgin, the features of the little Raphael. Although there is no evidence in this regard, it does not seem out of place to establish a link between the Cagliese church and the young Raphael. The work can still be admired on the left side of the chapel. In the first chapel on the left is the painting of Il Miracolo di Soriano, from the Neapolitan school for which the name of Cesare Fracanzano was recently made. In the first chapel on the right, the painting by Gaetano Lapis with the Presentation at the temple is worthy of mention. Next to this, on the right, there is, within a niche, the fresco with the Annunciation whose attribution, rather debated, has recently been attested on Timoteo Viti. The crypt, below the presbytery, preserves a fresco decoration by Antonio Viviani called il Sordo, among the students of Federico Barocci. Here it will be buried between Michelangelo Nanni, who died in the concept of holiness in 1671, promoter of the seventeenth-century renovation of the building.
